Os Gêmeos* ("The Twins" em inglês) é o boss exclusivo da dificuldade Hard Mode, que é essencialmente uma versão mais difícil do Olho de Cthulhu. Os gêmeos são duas entidades voadoras separadas, Retinazer e Spazmatismo, conectadas por uma gavinha, cada uma com seu próprio padrão de ataque e contagem de vida. Ambos devem ser mortos para derrotar o chefe. O retinazer é geralmente aquele que é derrotado por último, porque geralmente é mais difícil de atingir graças aos seus lasers. Os gêmeos podem ser convocados manualmente usando um olho mecânico à noite (19h30 às 4h30). Eles também têm 10% de chance de desovar aleatoriamente ao anoitecer, uma vez que um Demônio ou Altar Carmesim tenha sido destruído, desde que os Gêmeos ainda não tenham sido derrotados pelo menos uma vez no mundo atual, com sua chegada anunciada pela. status message "Esta noite vai ser terrível...".

Existem dois olhos: Retinazer e Spazmatism. Retinazer é um olho que atira a laser com uma íris vermelha. O espazmatismo é um olho de textura semelhante, embora tenha uma íris verde. Ele dispara Chamas Amaldiçoadas verdes.

Ataques[]
Primeira Fase[]
- O globo ocular com a íris vermelha, Retinazer, dispara lasers roxos de alta velocidade. Geralmente tentará permanecer diagonalmente acima do jogador.
- O globo ocular com a íris verde, Spazmatism, dispara bolas de média velocidade de Chama Amaldiçoada. Geralmente, ele tentará permanecer alinhado horizontalmente com o jogador.
Ambos alternam entre seus ataques à distância e ataques corpo a corpo de carga, bem como o Olho de Cthulhu. O ataque de carga do Retinazer tem maior velocidade, mas tem períodos mais longos entre as cargas e faz menos cargas do que o Spazmatismo.
Segunda Fase[]
Cada olho tem uma segunda fase, que acontece individualmente quando a vida deles desce a 40%. O Retinazer transforma em 8000 / 12000 / 15300 de vida enquanto o Spazmatismo transforma em 9200 / 13800 / 17594 HP.
- Retinazer becomes a mechanical eye with a laser cannon in its pupil, and only fires projectiles, ceasing its attempts to ram the player (or align with the player diagonally). It fires more rapidly and with higher damage, and periodically fires in quick barrages which do significantly less damage. The barrages become more intense and frequent the more damage Retinazer takes, eventually occurring non-stop.
- Retinazer will, however, align horizontally with the player when firing barrages, though not as tightly as first-phase Spazmatism does.
- Spazmatism becomes a mechanical mouth (similar to the Eye of Cthulhu) that chases the player, charging with greater speed and range, and periodically spews streams of Cursed Flames from its mouth in a manner similar to the Flamethrower. The flames can inflict the Cursed Inferno debuff and deal high damage, so it is a good idea to use healing potions in this part of the fight. It no longer attempts to align horizontally with the player.
In Expert Mode, Retinazer's laser firing becomes faster and charges cover more distance as its health depletes. Spazmatism's fireballs inflict Cursed Inferno for 2x / 2.5x the duration. Spazmatism also charges faster in its second form.

- When spawning randomly, The Twins spawn exactly 81 seconds after the status message has appeared. Being in a layer lower than the surface, or exiting the world (in singleplayer), will prevent The Twins from spawning randomly.

- Both eyes can drop their respective trophy when killed, regardless of which is killed first. It is possible for both trophies to drop in one battle.
- When one enters the second stage and its speed increases, it is possible to have The Twins despawn after killing one of the eyes. For this to happen, you must be moving fast enough to keep ahead of the second stage eye, such that the first stage eye falls behind (off screen).
- The Twins may despawn if they are offscreen too long, which can be a serious problem if you are attempting the Mecha Mayhem Achievement.

- Focus on defeating one eye completely first without doing too much damage to the other one. Battling The Twins while both are in their second form makes the battle much more difficult, especially in Modo Perito.
- Spazmatism may be a better choice for defeating first, since its cursed flames are difficult to avoid without proper mobility equipment/buffs in its second form. Retinazer is less deadly, as it cannot inflict debuffs and will not charge in its second form, which helps the player get some distance if they are a ranger.
- A Cross Necklace helps during The Twins' second form, as players will take far less damage during barrage attacks.
- The Twins are only immune to Confused, so using a weapon that inflicts a debuff may be useful during the fight.
- A good arena for this boss is simply a long, flat plane. With Lightning Boots (or better) and a ranged weapon, the player can continually outrun The Twins' attacks while firing back at them.
- The Sharanga is an excellent weapon against The Twins if it is fired as the player runs away.
- A Arco Tempestuoso de Dédalo with Holy Arrows can be a great tactic, as it can deal massive amounts of damage while staying out of range of many of The Twins' attacks.
- The Faca das Chamas das Sombras can be great for melee players, as it does not require any ammo or mana, and can hit both eyes if aimed correctly.
- Retinazer's second form cannot attack the player if they are hiding in a box. Defeating Spazmatism then causing Retinazer to reach its second form can become an almost guaranteed win if this strategy is used.
- A Gravitation Potion can be an excellent method of keeping distance from the Twins, allowing them to be attacked freely.
- Farming for the Black Spot on Master mode could help bypass making an arena and make dodging/keeping distance a lot easier during the fight.
Trivia[]
- Retinazer's name is a portmanteau of retina, a part of the eye, and laser. Spazmatism's name is a portmanteau of spasm, meaning to twitch uncontrollably, and astigmatism, an eye disorder.
- Spazmatism's second form has visible bolts along either side, whereas Retinazer’s second form only has them along one side.
- The Twins seem to have complete heterochromia iridum (two different eye colors).
- In its second form, Spazmatism's fire stream attack functions almost identically to the Flamethrower weapon, with similar range and damage, but inflicting the Cursed Inferno debuff instead of On Fire!.
- The Twins are the only boss with two trophies, and the only mechanical boss with phases.
- Despite inflicting it, Spazmatism is not immune to Cursed Inferno.
- The Twins are one of three bosses with a corresponding minion (Optic Staff), the others being the Disco Voador de Marte (Xeno Cajado) and Duke Fishron (Tempest Staff).
- In Modo Mestre, The Black Spot mount travels faster than The Twins, allowing the player to run away and fire backwards for the entirety of the fight.
